Allocated Funds Request Form
Policy on Disbursement of Allocated Funds:
Allocated funds are those funds that your team/activity/club have earned in association with the Lakeland Booster Club. As the coach or advisor, you are expected to spend your allocated funds responsibly and in the best interest of your team/activity/club. Your allocated funds are intended to help fund and fill in financial gaps with your team/activity/club. Allocated funds should be requested for similar needs described in the Grant Request policy, such as, equipment, supplies, uniforms, accessories, travel, awards, social events, or other needs for your team/club/activity.
In order to receive funds from your allocated funds' account, you must complete this Allocated Funds Request Form.
Once completed, you will be contacted by the Lakeland Booster Club Board with any questions, and notified of the status of your request. We do require at least 7 days notice of the need of a check or item purchased.
